truck mixin oil with fuel

my buddy's truck has been mixing oil and fuel and we changed the orings and its still mixin what could be causin it.

Did you check all the injector cups for any cracked or worn ones?

Did you lube the O rings heavily? Possible that one could have gotten rolled and not seating.
